Application to Graduate

Graduating - Request a Parchment, Certificate, or Replacement Parchment


1. Extension Certificate, Certificate, or Diploma

Upon completing your extension certificate (part-time program), certificate (full-time program), or diploma (full-time program), complete and submit an Application to Graduate form. The form notifies the Program Coordinator that you have completed your program. They will check your academic record and send you your parchment if you have met all requirements.
 

2. Certificate of Completion program

When you have finished a Certificate of Completion (part-time program), complete and submit a Request for Certificate form. The form notifies the Program Coordinator that you have completed your program. They will check your academic record and send you your certificate if you have met all requirements.
 

Submit completed Application to Graduate or Request for Certificate form to Continuing Education

3. Replacement Parchment

If you have lost your official parchment, please complete a Request for Replacement Parchment form.
 

Submit completed Request for Replacement Parchment form with payment to Continuing Education Registration Services
