Nursing and Midwifery Cardholder Agreement

Use of the Card:

The Nursing/Midwifery Practice Photo Identification Card is the official identification card for all Mount Royal University Nursing and Midwifery programs.  All nursing and midwifery students require this card to show they are a valid student in the Nursing or Midwifery programs.

 

You agree to:

  1. Display your photo ID card at all times when in the practice setting.
  2. Behave in a professional manner deemed appropriate within the nursing profession.
  3. Report lost or stolen ID cards immediately to the ONECard Office.
  4. Replace a lost or stolen nursing/midwifery ID card at your own expense for a fee of $5.00.
  5. Upon exit from the program under any circumstances, other than graduation, you must return the ID card to the OneCard Office.
  6. Your digital photograph being stored in University systems ( e.g. Card production, Residence, Security) and printed on the face of your ONECard.

You understand that: 

Your Photo ID is non-transferable and can only be worn by the individual displayed on the card. 

By accepting your Nursing/Midwifery Practice Photo Identification Card you agree to the conditions as outlined above. The School of Nursing and Midwifery, and the ONECard office, reserves the right to amend these policies without notice to you.

Privacy Policy:

The personal information that you provide is collected under the authority of the Freedom of Information and Protection of Privacy Act [section 33(c)] and the Post-Secondary Learning Act [section 65(a)].
